This is a daily waiver, please take the time to fill out the information below. All information must be accurate & filled out before getting tattooed.

By submitting this form, I acknowledge that I am voluntarily requesting tattoo services from The Pacific Tattoo Studio. I understand that receiving a tattoo involves certain risks and permanent results.

I Understand and Agree That:

I am at least 18 years old and have provided valid identification.
I am not under the influence of drugs or alcohol.
I do not have any medical conditions that may affect the healing or safety of the tattoo procedure (including but not limited to skin infections, open wounds, hemophilia, or communicable diseases).
I have informed my artist of any allergies, sensitivities, or medical concerns prior to my appointment.
I understand that tattooing involves needles and blood exposure, and there is a small risk of infection, allergic reaction, or scarring.
I acknowledge that healing time varies, and following proper aftercare instructions is my responsibility.
I understand that all payments are final and that deposits are non-refundable.
I release The Pacific Tattoo Studio, its artists, employees, and affiliates from any liability related to the tattoo procedure, healing process, or final outcome.
